We hereby propose to provide weekly pool service at the above address for:

 

( ___ )  $29 per week for weekly full service (Pools 10,000 to 15,000 gallons)

 

( ___ )  $34 per week for weekly full service (Pools 15,000 to 25,000 gallons)

 

( ___ )  $______ per week for weekly full service ____excess foliage and/or ____ under 10,000 gallons or over 25,000 gallons

 

Note: If your pool is greater than 25,000 gallons an additional charge of $1 for every 1,000 gallons will apply to the weekly service price quoted above. The prices above are based on average chemical usage on similarly sized pools. If your pool’s chemical consumption exceeds the maximum average allowed the additional chemicals will be charged accordingly. ($29 max avg. = 3 Cl tabs / wk & $34 max avg. = 4 Cl tabs / wk)

 

The following is included in weekly full service:

¨        Test & Adjust Water Chemistry (Free Chlorine, pH, Total Alkalinity are Checked Weekly.  Cyanuric Acid, Calcium Hardness & Salt Checked Monthly)

¨        ALL Chemicals are Included

¨        We Maintain Your Pool According to the Calcium Saturation Index & Health Code Standards

¨        Empty traps & skimmers weekly (We take ALL trash & debris waste with us when we leave.)

¨        Backwash filter monthly. (D.E. & Sand Filters Only. More frequent backwashing of sand filter is included in service price.)

¨        Net the pool / spa & negative edge trough

¨        Brush tile (waterline), walls & steps weekly & floors if needed (calcium & scum-line removal not included)

¨        Check the equipment for visible & audible problems

¨        Clean salt cell as needed

¨        Vacuum the pool / spa as needed (if pool requires vacuuming every week an additional charge of _$20_ per visit will apply)

¨        Lube Pump O-ring as needed

¨        Guaranteed Algae Prevention (If your pool starts to turn green we’ll fix it for free.  See website for complete details.)

¨        Leave Skimmer Tag after each visit under skimmer lid

 

*Per week billing will usually be generated and mailed on the 1st of each month.  The method for calculation is as follows: for example: 52 weeks per year divided by 12 months = 4.3333 weeks per month times the weekly service rate equals your monthly charge. (i.e. 4.3333 x 29 = 125.67)

 

If you have a cartridge filter, there will be an additional charge of _$90_ for filters 325 sq. ft. & under or _$125_ for filters over 325 sq. ft.. each time the filter is cleaned.  (NOTE: Cleaning will occur every 3 months or when the filter pressure rises 8-10 psi according to the manufacturer’s recommendations.  Please keep in mind that cartridges only have a life expectancy of 2 – 3 years at which time they will require replacement.)

 

If you have a D.E. filter, there will be an additional charge of _$95_+ cost of grids if needed_ each time the D.E. filter grids are cleaned.  (NOTE: Cleaning is performed 1 time per year, twice per year on pools with heavy usage (pets, parties, etc.) or high foliage/debris levels.)

 

¨   I authorize that minor work/repairs totaling _$100.00_ or less can be performed without my prior authorization. I also authorize salt cell cleaning to be performed quarterly or as needed at the price of _$35_ per cleaning. All repairs in excess of the amount listed require the customer’s prior approval. All repairs are C.O.D. unless other arrangements are made in advance. All old parts & materials will be left onsite.

¨   There will be a charge of _$23.89_ for a high capacity floating chlorinator unless there is a suitable chlorinator present.

¨   I will let the technician know of any parties or planned activities that may result in a need for extra pool service or chemicals, one week in advance.

¨   I understand that I must provide clear and easy access to the pool and pool equipment.  I understand that additional charges, for trimming plants or removing debris, may be assessed, or I may be charged for service that cannot be completed.

¨   I understand that any additional services or repairs not specifically listed on this agreement ARE NOT INCLUDED, and that this agreement is transferable, and that prices are subject to change upon notice.

¨   I understand that Arizona Pool Pros does not provide service or chemicals for above ground spas or hot tubs that do not share the water or circulation system of the swimming pool.  Please refer to the New Client  Packet or www.azpoolpros.com for more information.

¨   Customer Responsibilities:

o         Maintain the water level.

o         Keep all trees and plants trimmed away from the pool area and pool equipment.

o         Trim all palm tree pollen pods within 2 weeks AFTER they sprout.

o         Keep all equipment in good operating condition.

o         Run the pump/filter during the proper times. (Summer months usually requires some daylight hours.)

o         Run the pump/filter for the proper number of hours. (Sand filters usually require at least 10 hours during the summer.)

 

The following terms and conditions apply to the service:

 

1.       Agreement Term: An agreement for service is for a time period not less than one year from the date client first receives service.  If client cancels service(s) within the first 35 days after the start of service they will not be charged with an early termination fee.  However, if client cancels during the remaining time period of the service agreement an early termination fee of _$250.00_ will be added to their final bill.  **I understand that this service agreement is automatically renewed every April 1st for an additional year, unless I contact the service company, in writing, to discontinue service in March. If client cancels service after payment has been made, Arizona Pool Pros will complete the paid for work and no credits or refunds will be given.

2.       Payment Terms: The customer agrees to pay all balances within 21 days of the billing date. All billing statements are presumed correct unless you notify us IN WRITING within 21 days of the billing date. The late charge is currently $15.00/month (to cover additional billing expenses) plus 1-3/4% per month interest on the unpaid balance (annual % rate = 21%). I understand that any account balance left unpaid past 60 days will be turned over for collections. I, the customer, agree to pay any and all reasonable attorney’s fees and collection expenses plus reimbursement for the Time of “Arizona Pool Pros” at the standard hourly repair rate ($90.00/Hour). (A minimum of a $200.00 fee and an additional $30.00 per month will be charged for any account with an unpaid balance past 60 days overdue.) I, the customer, also agree to pay an additional $55.00 fee for all check(s) returned unpaid (i.e. stop payment, insufficient funds, etc.). NOTE: I, the undersigned client understand and agree that, in the event of a need for us to place a lien on your property, the lien will not be removed until any and all additional late fees and charges listed here that have accumulated since the lien have also been paid.

3.       Safety Issues: The homeowner/resident is responsible to maintain code compliance with the city on issues such as fencing, gates and all other safety issues.  Arizona Pool Pros employees will not perform service if anyone under the age of 18 is without parental supervision in the pool area. Should this occur and service cannot be performed, NO refund or credit will be given.

4.       Chemicals: All chemicals are provided by us and stored on our vehicles.  This includes chlorine, acid, salt, other balancing chemicals, algaecides and phosphate remover.  A preventative care dosage of phosphate remover & algaecide is added every 8 weeks. Storms may increase the demand for these two chemicals which will be charged additionally.  Any plaster treatment chemicals are not included.

5.       Stormy Weather & Lightning: If stormy weather occurs, (i.e. rain, wind, or threat of lightning) on the day service is performed, chemicals will be added and baskets will be cleaned.  Services, beyond this will not be performed until your next service date.  In the event of lightning, we will follow the 30 / 30 Rule. 6.       Schedule: Our technician performs the service on the same day each week. I understand that in the unlikely event that my service visit needs to be rescheduled due to a technician’s (holiday, illness, vehicle, pool service & repair seminars, etc.), my service may be provided as soon as 5 days, or as late as 9 days from the day the pool was last serviced. Also, due to illness, or death in the family, it is possible that service may be missed at any time during the year. Your pool will remain in good condition, even if it is not serviced every week. 7.       Pets: The homeowner/resident is responsible to contain and restrain their pets. Pets should not be allowed to swim in the pool as this will cause an increased demand in chemical consumption (i.e. the chlorine consumption for one medium sized dog is equivalent to 4-5 average sized people swimming in the pool.) Clients who allow their pets to swim in the pool will be charged for the additional chemicals & repairs that may be needed to fix chemical imbalances or damage resulting from fur getting into filters, vacuums, etc.

8.       Gates: Arizona Pool Pros employees always make certain that all gate(s) leading to the yard and pool area are properly shut, latched, (locked when applicable) prior to leaving the property. In the event that your pool has any type of cover or safety net on the pool the day service is performed, chemicals will be added and baskets will be cleaned only. Removal of covers or nets is the responsibility of the client.

9.       Customer Satisfaction: Our goal is your complete satisfaction.  In the event that a job is not satisfactorily completed, it is the client’s responsibility to notify Arizona Pool Pros within 7 days (preferably sooner), so that we can look into the matter and resolve it quickly.  If you do not bring the issue to our attention during that time frame, then we cannot issue any credits.

10.    Additional Cleanup Charges: Clients receive 2 Monsoon Cleanups per year. Refer to website for details. In the event that the pool is unusually dirty at time service is started or in the event that the pool receives excessive debris and/or dirt load due to landscaping, foliage, etc, there will be an additional charge of _$20_ for every additional 15 minutes over your pools average service time, if cleanup is done during regularly scheduled service. If client requests cleanup for a day other than their regularly scheduled service day, then the _$90_ per hour (one hour minimum) service charge will apply.

11.    Patio Furniture & Toy Removal: Removal of patio furniture and toys from the pool is the homeowner/resident’s responsibility.

12.    Repairs: We are happy to perform all repairs and most new product installs for your pool.  Call us for pricing.  Our labor charge to dispatch a technician to your pool is _$90_ per hour (one hour minimum).  This is charged on all service calls unless other arrangements are made at the time the service call is requested.  If you feel that a service call should not be charged for any reason (warranty, etc.), please make this clear when calling in a service request.  Otherwise the service call will be charged at full rate.

 

¨        I agree that no oral representations have been made to me contrary to the understanding set forth herein.
